Lack of exercise, not diet, linked to rise in obesity

According to a July 7th Stanford Medicine News article, "An examination of national health survey results suggests that inactivity, rather than higher calorie intake, could be driving the surge in obesity. Inactivity rather than overeating could be driving the surge in Americans' obesity, according to a study by a team of Stanford University School of Medicine researchers. (Obesity, Abdominal Obesity, Physical Activity, and Caloric Intake in U.S. Adults: 1988-2010) Examining national health survey results from 1988 through 2010, the researchers found huge increases in both obesity and inactivity, but not in the overall number of calories consumed.
"'What struck us the most was just how dramatic the change in leisure-time physical activity was,' said Uri Ladabaum, MD, associate professor of gastroenterology and lead author of the study. 'Although we cannot draw conclusions about cause and effect from our study, our findings support the notion that exercise and physical activity are important determinants of the trends in obesity.'..."

An honest discussion about "the fattest people in the world"
That's the title of an article at 350.org about having a conversation about our obesity problem in the U.S. As we've built a transportation system that forces people to rely on cars and reduces opportunities for walking and biking, we as a nation have gotten fatter. There are likely many causes of the problem but reduction in walking and biking, especially among children, certainly plays a major role.
Yes, let’s have an honest discussion about being the “fattest people in the world.” Every day, “decision makers” make transportation infrastructure and planning decisions that favor cars over people, decisions that make walking and biking—walking or biking to real public transit options, to work, to school, to the store—come to seem more and more…radical. It is indeed time we took responsibility for these and for the full range of consequences they entail. Let’s take some personal responsibility and make sure we as a society make the decisions that instead favor people over cars, favor transportation infrastructure built around the human scale.

Eat to ride or ride to eat?

I appreciated the information given in Kathleen Parker’s May 9 op-ed column, “Eating our way into a disaster.” It did, however, only consider one side to the thinness issue by discussing caloric input of high-glycemic food. I’m a 49-year-old woman, 5-foot-6 and 113 pounds. I eat and drink whatever I want, taking in more than 2,000 calories a day. I’m able to maintain my low body weight because I gave up driving two years ago in disgust over the BP oil spill disaster.
I ride my bike for all of my personal transportation, typically cycling 150 miles a week. The mileage I previously covered in a heavy, gas-powered car is now provided by my leg muscles; I burn a lot of food.
While most people are filling up their gas tank, I’m filling up my plate, including lots of bread and potatoes. Americans indeed eat too much food and eat too thoughtlessly. Yet, if they would get out of their cars, and walk or cycle instead, they wouldn’t be so fat.

The letter reminded me of a day last week when I had a craving for a pecan sticky bun. It must have contained 1,000 calories but I knew I was riding into DC and back the next day. The bun was my breakfast, then for lunch I ate a fry bread grill cheese, an apple tart, and some fruit at the National Museum of the American Indian cafeteria. Granted I overdid it a bit that day. However, most people who bike often, riding on some days much longer than on others, learn to listen to their bodies and to regulate their calorie intake accordingly. When expecting to burn more calories we can splurge a little and eat some normally forbidden foods.
I often recall an overweight friend who said "It's very simple, burn more calories than you take in and you'll lose weight." While it's not quite that simple, when discussing the obesity epidemic we need to remember that the problem involves much more than just nutrition.
In case you're wondering, I'm 5'9" and weigh around 145 pounds.

Childhood obesity conference

We met many people who were interested in trying to get more kids to walk and bike to school and for short trips in their neighborhoods. They need better bike education and they need better facilities. The Safe Routes to School program is a comprehensive way to address both issues and it was mentioned many times during the conference. It will take time to transform our car-dependent communities into walkable/bikeable areas, but it's possible and we need to start now.Labels: obesity, safe routes to school

FABB at Clean Fairfax Earth Day/Arbor Day
It was a wet day at the Fairfax Co Government Center for Clean Fairfax Earth Day/Arbor Day. Fortunately the organizers moved the event inside. As a result of the rain there were many fewer visitors than expected. Despite the rain several people stopped by the FABB table and picked up maps and other bike literature, signed up for the FABB newsletter, and talked about riding in Fairfax. Many people said they would ride more if conditions were better. It's a common theme when we are out talking to the general public. People say they want to use their bikes but they don't feel safe riding on the roads. Most of our streets were not designed for cyclists, and novice cyclists feel especially vulnerable. The main reason we teach the Cycling with Confidence class is to help cyclists learn the skills needed to ride in traffic. The county needs to do much more to encourage people to ride by providing safe, convenient bike facilities.
We had a good conversation with the representative from Inova who was at the table next to us. Inova is sponsoring Let's Move the Needle on Childhood Obesity, and we discussed the importance of kids using active transportation. It's a lifelong activity and helps them become independent at an earlier age. Kids who learn about the rules of the road when young become better drivers as they get older. They also learn they don't have to drive everywhere to get around. We plan to have a table at the event and hope to meet others who want kids to be able to use their bikes for transportation and to get exercise.

Does Driving Cause Obesity?
That's the title of a Freakonomics article in the NY Times about a study of people who take public transit:People are significantly fatter in countries, states, and cities where car use is more common. Mass transit use, on the other hand, is correlated with lower obesity. But there has been scant evidence that public transportation actually causes widespread weight loss — until now. A study of residents in Charlotte, N.C., found that users of the city’s new light rail system were 81 percent less likely to become obese, and reduced their Body Mass Index by an average 1.18 points — the equivalent of 6.45 pounds for a person 5’5″ tall. The study appears in the American Journal of Preventive Medicine.
And by extension is bike commuting one remedy? That's the conclusion of a 2008 study, Walking, cycling, and obesity rates in Europe, North America, and Australia:RESULTS: Countries with the highest levels of active transportation generally had the lowest obesity rates. Europeans walked more than United States residents (382 versus 140 km per person per year) and bicycled more (188 versus 40 km per person per year) in 2000.
DISCUSSION: Walking and bicycling are far more common in European countries than in the United States, Australia, and Canada. Active transportation is inversely related to obesity in these countries. Although the results do not prove causality, they suggest that active transportation could be one of the factors that explain international differences in obesity rates.

More bad health news about kids
When will parents realize that driving their kids everywhere, in part because they fear for their safety, is doing more harm to their health and well-being than good? The headline says it all: High cholesterol puts 1 of 5 teens at risk of heart disease:"This is the future of America," said Linda Van Horn, a professor of preventive medicine at Northwestern University who heads the American Heart Association's Nutrition Committee. "These data really confirm the seriousness of our obesity epidemic. This really is an urgent call for health-care providers and families to take this issue seriously."
While exercise is only part of the equation, lack of exercise certainly plays a role in development of high cholesterol and childhood obesity.Because of the obesity epidemic, some have said that this is the first generation in which parents may outlive their children. Exacerbating the problem is the amount of time children spend in front of a computer, tv, or some other media device: "children ages 8 to 18 devote an average of seven hours and 38 minutes a day consuming some form of media for fun." Walking or biking to school is a simple, low cost part of the solution.

Providing bicycle infrastructure for combating childhood obesity
Local governments should do a better job of providing good bicycle and pedestrian facilities so that children can get more exercise, according to Local Government Actions to Prevent Childhood Obesity, a new study that was released today. The number one goal for increasing physical activity is to "Encourage walking and bicycling for transportation and recreation through improvements in the built environment" and the first action step is to "Adopt a pedestrian and bicycle master plan to develop a long-term vision for walking and bicycling in the community and to guide implementation." pp. 57 & 58.See the Yahoo! news article Tax junk food, drinks to fight child obesity, and the USA Today article Report maps out solutions to child obesity.
While some progress is being made in Fairfax County, the schools have not done enough to ensure that kids can safely walk and bike to school. Perhaps we should have an "adopt a school" program so that bike advocates can work with a single school to encourage them to provide bike racks to kids and to provide better bicycle education. More on this topic in a later post.

How to fight obesity: ride a bike
It shouldn't come as a surprise that one consequence of creating a car-dependent society is the lack of opportunities for most people to get exercise on a daily basis. When most of our transportation dollars are spent building roads for cars, there is very little available for more active modes of transportation like walking and biking. One result: we spend $147 billion a year treating the obese.That is the finding of a recently released Centers for Disease Control report entitled Annual Medical Spending Attributable To Obesity. A startling finding is that "obesity rates increased by 37 percent between 1998 and 2006 (from 18.3 percent to 25.1 percent of the population)".
The take-home message is that without a strong and sustained reduction in obesity prevalence, obesity will continue to impose major costs on the health system for the foreseeable future. And although health reform may be necessary to address health inequities and rein in rising health spending, real savings are more likely to be achieved through reforms that reduce the prevalence of obesity and related risk factors, including poor diet and inactivity. These reforms will require policy and environmental changes that extend far beyond what can be achieved through changes in health care financing and delivery.
What environmental changes are needed? The CDC provides some guidance in a Morbidity and Mortality Weekly Report published this week entitled Recommended Community Strategies and Measurements to Prevent Obesity in the United States. The report lists specific recommendations for preventing obesity. Number 17 is Communities Should Enhance Infrastructure Supporting BicyclingOverview
Enhancing infrastructure supporting bicycling includes creating bike lanes, shared-use paths, and routes on existing and new roads; and providing bike racks in the vicinity of commercial and other public spaces. Improving bicycling infrastructure can be effective in increasing frequency of cycling for utilitarian purposes (e.g., commuting to work and school, bicycling for errands). Research demonstrates a strong association between bicycling infrastructure and frequency of bicycling.
Evidence
Longitudinal intervention studies have demonstrated that improving bicycling infrastructure is associated with increased frequency of bicycling (104,105). Cross-sectional studies indicated a significant association between bicycling infrastructure and frequency of biking (p<0.001) (103,106,107).
Suggested measurement
Total miles of designated shared-use paths and bike lanes relative to the total street miles (excluding limited access highways) that are maintained by a local jurisdiction.
This measurement captures the availability of shared-use paths and bike lanes, as defined by the American Association of State Highway and Transportation Officials, relative to the total number of street network miles in a community. The numerator of this measurement includes both shared-use paths and bike lanes. The denominator of this measurement is limited to paved streets that are maintained by city/local government, and excludes limited access highways. Although no estimated standard exists for this measurement, data collected from local governments reporting on this measurement can lead to establishment of a standard.
